Role Description
Working closely with health and Social Care colleagues to support the complex discharges out of the acute hospital to ensure the level of service is appropriate to meet their needs.
Provide a holistic approach to interventions, working with the whole person, their family or system, building relationships and networks, enabling individuals, their families and carers to achieve sustainable good outcomes. Have a good understanding of KCC law and procedures or know how to access this information/advice if required.
Support and participate in, joint and integrated working with a range of teams including community safeguarding teams, partner organisations and other parties to offer a broad range of options to clients and carers to keep them safe and reduce risk. Join wash up discussion within health to encourage sharing of knowledge and information.

Responsibilities

Requirements

- Assess, plan, agree, implement, and evaluate care for patients, recognizing your limits and escalating concerns.
- Maintain clear and accurate records of advice provided to families, observations, and actions taken.
- Respond to changes in a patient's condition.
- Comment on and participate in audits and surveys for proposals of change.
