Neonatal Transitional Care and Community Outreach Band 6 Junior Sister/Charge Nurse
An exciting opportunity for experienced neonatal nurses to develop their leadership skills and experience in a Neonatal Transitional Care and Community Outreach Band 6 Junior Sister/Charge Nurse post.

This post is pivotal within the unit nursing structure.
We are looking for dynamic individuals who can adapt their neonatal skill set to the postnatal ward setting and the community. The Transitional Care team is an exciting service which involves the team providing a model of transitional care to babies, who are being nursed alongside their mothers within the maternity service. This supports reducing newborn separation and the maintenance of the family unit. It involves working across both neonatal and maternity unit boundaries and with the wider MDT. The Saint Mary’s MCS Neonatal Outreach service involves visiting families within their homes, supporting clinics, meeting with families before discharge to facilitate a high quality effective and efficient transition from hospital to home.

Main duties of the job
- Professionally attend babies being discharged from the neonatal unit and being cared for at home, or those babies with a special care dependency on the post-natal wards.
- Promote health education activities to parents and families.
- Liaise with the multidisciplinary and multi-agency teams to ensure that babies and their families receive a high quality service, coordinating and participating in case discussions as required.
- Lead by example and act as a positive role model for all staff, taking responsibility for personal and professional development.
- Maintain personal standards of conduct and behaviour, which are consistent with Trust and Nursing and Midwifery Council (NMC) standards and requirements.
- Ensure all documentation is accurate, contemporaneous and complies with NMC standards.
- Support the MNORT Clinical Lead and Matron for Continuing Care in relation to the transformation agenda within the clinical setting, including effective leadership, the setting, monitoring and maintenance of professional standards, collaborative working, the management of complaints and leading the nursing contribution to quality and safety at a department/unit level.
- Contribute to formulating policies and strategies within area of practice.
- Demonstrate awareness of legislation regarding children and families, for example safeguarding issues and act appropriately when required.
