Salary: £46,142 - £51,356 per annum
Position: Permanent, Full Time
Hours: 37 hours per week
Department: Adult Social Care
About the Role
An exciting opportunity has arisen for an experienced and enthusiastic Transitions Manager to lead a dedicated Young Adults Service team within Adult Social Care.
This is a key leadership role responsible for managing a busy and committed team supporting young adults aged 18–25 through key life transitions, promoting independence, achieving positive outcomes, and enabling individuals to live fulfilling lives within their communities. The post holder will provide leadership, professional oversight, and operational management to Social Workers, Senior Social Workers, and Social Care staff, ensuring the delivery of high-quality, strengths-based, and person-centred services.
Key Responsibilities
- Oversee workforce performance, supervision, safeguarding practice, assessments, support planning, and reviews
- Ensure services are delivered in accordance with legislative requirements, best practice, and Lancashire County Council's values
- Support young adults with a range of needs, including learning disabilities, autism, physical disabilities, mental health needs, and complex circumstances
- Work collaboratively with education providers, health services, employment and training agencies, community organisations, families, carers, and partner agencies

Requirements
- Qualified Social Worker with substantial post-qualifying experience
- Excellent knowledge of Adult Social Care legislation, transition processes, and safeguarding responsibilities
- Proven ability to lead, motivate, and develop staff
- Commitment to promoting strengths-based practice, managing risk proportionately, supporting continuous improvement, and delivering high-quality outcomes for young adults
