Home Support Matters - Triage Care Worker

Are you an experienced and confident care professional who can make quick, effective decisions? We’re seeking skilled and compassionate care workers to join our team, delivering high-quality care during peak demand periods.

What You’ll Do

Play a critical role in supporting new customers integration, ensuring excellent standards of care from day one of a customer’s journey with HSM.

Your Responsibilities Will Include

  • Providing short-term care and support, focusing on new customer integration.
  • Building strong rapport with customers and their families from the outset.
  • Identifying risks, safeguarding concerns and any required interventions.
  • Communicating clearly with branch teams, field care supervisors and other professionals.
  • Ensuring accurate, timely documentation that meets regulatory standards.
  • Representing Home Support Matters with professionalism, empathy and expertise.

What You’ll Bring

  • Strong experience in care or support roles.
  • Confidence in assessing needs and making informed decisions.
  • Excellent communication and calm under pressure.
  • Solid understanding of safeguarding and risk.
  • A proactive, organised and professional approach.
  • A Full UK Driving Licence and access to own vehicle with business insurance.

Minimum Availability Must Include

07:00 - 13:30 AND/OR 16:00 - 22:00

With a requirement to work every other weekend or every Saturday/every Sunday.
