Exciting Career Opportunity: Band 6 Triage Clinician

An exciting career opportunity has arisen for a Band 6 Triage Clinician to join the Bracknell Community Nursing Team – this role is both office and practice-based.

About the Role

The Triage Clinician will provide clinical support, assistance, and professional expertise to all callers to the Clinical Nursing Triage, including both healthcare professionals and patients within Bracknell.

The service is the main point of access for Bracknell Community Nursing, taking referrals from professionals and patients requiring healthcare services and the Trust Health Hub.

Key Responsibilities

  • Provide clinical support, assistance, and professional expertise to all callers.
  • Demonstrate sound clinical reasoning and have a positive, enthusiastic approach to the role.
  • Work partially office-based and with patients in their own homes as part of the urgent nurse support.
  • Support one of our community clinics on-site in Bracknell.
  • Communicate with patients and service providers of varied needs.
  • Offer sound expert clinical advice that oversees entry into our service.
  • Maintain up-to-date Statutory and Mandatory training and be prepared to rotate into clinical services as necessary to maintain clinical skills.
  • Hold professional registration and adhere to professional codes of practice and conduct.
  • Work in conjunction with administrative support staff and the community nursing service to provide high-quality, clinical triage using knowledge, skills, critical thinking, and professional judgement.
  • Assist the administrative support staff where required by accepting the transfer of cases requiring additional clinical advice, reviewing recorded clinical information, and offering advice.
  • Provide urgent clinical support to patients in their own homes and in-house clinic support.
  • Control and manage calls effectively by using skilled questioning, active listening, and excellent verbal communication skills in a calm, professional manner.
  • Liaise and communicate clinical information verbally and/or via telephone to Clinical Nursing Staff as appropriate to ensure continuity of care for the caller.
  • The Community Triage Nurse will possess the ability to prioritize tasks, cases, and situations appropriately to ensure effective delivery of services during periods of high demand.
