Are you an organised, proactive and patient-focused individual looking for an opportunity to make a real difference within Community Nursing Services?

We are seeking a Triage Coordinator to join our busy and innovative Neighbourhood Triage Hub, providing a central point of contact for patients, carers, healthcare professionals and partner agencies.

This is a pivotal role within the service, helping to ensure patients receive the right care, at the right time, from the right team. Working closely with Community Nurses, Clinical Leads and members of the multidisciplinary team, you will support the coordination of referrals into the Community Nursing Response Service (CNRS), facilitate effective communication between services, and contribute to the smooth and efficient operation of the hub.

As the central non-clinical point of coordination, you will play a key role in supporting the triage and allocation of urgent referrals, maintaining accurate patient information, coordinating resources and ensuring the timely deployment of response teams. Your ability to manage competing priorities and respond effectively in a fast-paced environment will directly support the delivery of responsive, patient-centred care in the community.

We are looking for someone who is highly organised, calm under pressure and confident communicating with a wide range of people. You will have excellent administrative and coordination skills, a proactive approach to problem-solving, and the ability to work collaboratively as part of a dynamic team committed to delivering high-quality care closer to home. Whilst this role is advertised as an administrative role, we welcome applications from clinical staff.

What You'll Be Doing

  • Receiving and processing Urgent Community Response calls into Community Nursing Response Service.
  • Supporting the clinical triage process by gathering, recording and sharing relevant information.
  • Acting as a first point of contact for patients, carers and healthcare professionals.
  • Coordinating workloads and appointments to support timely and effective service delivery.
  • Liaising with community teams, acute services, GP practices and external partners.
  • Maintaining accurate patient records and ensuring information is handled confidentially and appropriately.
  • Monitoring and escalating urgent or complex issues to the appropriate clinical staff.
  • Supporting the hub to achieve performance targets and provide an excellent patient experience.

What We're Looking For

We are looking for an individual who:

  • Has excellent communication and interpersonal skills.
  • Is highly organised and able to prioritise competing demands.
  • Works effectively under pressure within a busy environment.
  • Demonstrates strong administrative and coordination skills.
  • Has experience of working within a healthcare, community or customer-focused setting.
  • Is confident using electronic patient record systems and Microsoft Office applications.
  • Can work both independently and as part of a multidisciplinary team.
  • Is committed to delivering high-quality, patient-centred care.

Our Neighbourhood Triage Hub is at the heart of community services, ensuring referrals are assessed efficiently and directed to the most appropriate care pathway. The team works collaboratively to provide a responsive service, supporting patients to remain safe and independent within their own homes and communities.
