Role Overview
Are you a passionate, and caring nurse with at least one year's experience in community looking to progress and develop leadership skills in a dynamic community nursing team?
If so, then we would like to meet you.
About the Role
A vacancy has arisen within the Unplanned Care Triage Hub, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team; and liaise with the wider multi-disciplinary team.
Key Responsibilities
- Provide triage assessment, clinical support, assistance, and professional expertise to patients, carers, and health and social care professionals accessing the Community Nursing Service.
- Support the Triage Hub team leader with the daily service coordination and delivery; and is pivotal in the communication within the team.
- Provide expert clinical support and guidance to other members of the Triage Team, and those accessing the service.
- Deputise in the absence of the team leader.
- Responsible for the assessment of health needs, prevention of hospital admissions, and facilitate early discharges from hospital.

Working Hours
The current hours of work are Monday to Friday 8am to 4pm, however, these will alter to a 7 day per week service, 08:00 – 20:00 as the service expands.
