Triage Pharmacist
The Clinical HCP Consultation and Triage Healthcare Professional is responsible for conducting structured telemedicine consultations and triage assessments for patients seeking access to Cannabis-Based Medicinal Products (CBMPs). This role focuses on gathering essential clinical history, screening for safety and eligibility, assessing safeguarding risks, and identifying clinical red flags to ensure appropriate escalation to the specialist prescribing team.
Location: On-site - Bishop Stortford

Pay Rates: Up to £30 per hour Ltd
Hours / Working Days: 37.5 hours per week / Monday to Friday, 8hr shifts between 8am –6pm

Core Duties & Key Responsibilities
Patient Triage & Clinical Consultation
- Conduct structured triage appointments with patients in accordance with protocols and clinical governance pathways.
- Clearly explain the HCP Consultation process, legal requirements, patient responsibilities, and restrictions surrounding CBMP pathways.
- Administer and document structured eligibility criteria questions.
- Discuss general health issues relevant to triage, patient safety, and CBMP suitability.
- Provide clear, factual, approved information to patients while maintaining strict boundaries—avoiding advice or assurances outside the authorised scope of practice.
Clinical Data Gathering & History Taking
- Systematically gather and document comprehensive patient histories, including:
- Current symptoms, past treatments, and current medical regimens
- Full medication history, allergies, and drug sensitivities
- Mental health history and substance use history
- General health factors impacting safety and treatment suitability

Risk Assessment, Safeguarding & Escalation
- Conduct structured clinical risk assessments aligned with guidance, professional standards, and company protocols.
- Screen for adult and child safeguarding concerns, vulnerability factors, mental capacity, consent, domestic abuse/coercion indicators, and risks to dependants.
- Identify clinical red flags, contraindications, cautions, incomplete health records, or potential risks requiring further review.
- Promptly escalate patient safety concerns, clinical risks, or information gaps to specialist clinical and prescribing teams.
Clinical Governance & Documentation
- Complete concise, accurate, and timely consultation notes, triage records, screening logs, and escalation reports.
- Maintain clinical records to support safe handovers, continuity of care, and compliance with data governance standards.
- Participate in clinical governance activities, including audits, incident reviews, complaint investigations, and quality assurance evaluations.
- Liaise with administrative and clinical teams to streamline appointment scheduling, patient coordination, and clinical handovers.
Boundaries & Activities Outside Scope
- Non-Prescribing Role: This role does not involve prescribing CBMPs, initiating or varying treatment plans, or making final clinical suitability decisions.
- Specialist Decision-Making: All prescribing, treatment initiation, variation, and final suitability determinations remain strictly within the remit of the authorised prescribing and specialist clinical team. (Note: Separate written authorisation and appropriate registration are required to undertake prescribing functions).

Professional Registration, Qualifications & Experience
Mandatory Requirements
- Active Professional Registration: Unrestricted registration with an authorised UK regulatory body (e.g., NMC, GMC, HCPC, GPhC, or equivalent).
- Right to Practise: Current right to practise in the UK with evidence of good standing, revalidation, or equivalent professional compliance.
Essential Experience & Skills
- Proven experience in patient-facing clinical healthcare, triage, risk assessment, and safeguarding protocols.
- Demonstrated ability to deliver safe care within a telemedicine or remote consultation setting.
- Excellent verbal and written communication skills, with the ability to interact professionally with patients, multidisciplinary clinicians, and operational teams.
- Strong clinical documentation skills and familiarity with clinical governance framework adherence.
- High level of attention to detail regarding patient safety, clinical red flags, and risk escalation pathways.
