Carer Support Officer (Carers Assessments) | Liverpool Carers Centre
Supporting unpaid carers across Liverpool through a person-centred assessment process, helping them identify their needs, improve their wellbeing, and access appropriate services and support.
Essential Requirement
Due to the community-based nature of this role and the need to undertake home visits across Liverpool, the successful candidate must hold a full UK driving licence and have access to a vehicle for work purposes.
Key Responsibilities
- Conducting Carers Assessments and developing personalised support plans using a strengths-based, "What Matters to Me" approach.
- Providing practical and emotional support to unpaid carers through home visits, community outreach, and one-to-one interventions.
- Signposting and connecting carers to health, social care, community, and voluntary sector services.
- Building effective partnerships with statutory agencies, healthcare professionals, and community organisations to improve outcomes for carers.
- Supporting carers to access resources, support packages, and opportunities that enhance their health, wellbeing, and resilience.
- Maintaining accurate case records, monitoring outcomes, and ensuring compliance with GDPR and safeguarding requirements.
- Producing KPI reports and contributing to service development, quality improvement, and organisational learning.
- Promoting carers' rights, raising awareness of available support, and representing the service at meetings, events, and stakeholder forums.
