Job Opportunity: Urgent Care / Intermediate Care Clinical Nurse
An exciting opportunity has arisen for an Urgent Care / Intermediate Care Clinical Nurse to work part-time (22.5 hours per week) for 12 months covering maternity leave.
You will be working within an integrated team, working across the community, this includes:
- Urgent Care Response Team
- Intermediate Care
- Covering the rehab unit at Jack Sears if required
This is a fast-paced, exciting position with a focus on admission avoidance, enabling hospital discharge, and providing short-term nursing input to patients while under intermediate care.
You will be based primarily at Kings Ash House in Paignton; however, you may be providing visits to patients across the whole of Torbay. A full driving license and access to a car is imperative for the role.
Key Responsibilities
- Delegating work to non-registered colleagues and having a supervisory role within the team.
- Planning and implementing care/treatment to ensure safe and effective delivery of care. This may require placing the patient in a place of safety or arranging immediate care support in their own home.
- Undertaking holistic, comprehensive assessments including both risk assessment on behalf of the team and nursing assessments involving a thorough examination of the patient’s medical, social, and emotional state to determine the most appropriate plan of intervention.
- Providing ongoing assessment of nursing needs of patients and carers in the community.
- Appropriate delegation of tasks to members of the team within their scope of practice.
- Working as an autonomous practitioner in the planning and implementation of nursing treatment programs.
- Advising and instructing carers and patients on health and well-being issues, and self-care strategies.
- Carrying out specialist and shared complex nursing assessments, to include continuing and palliative care.
- Developing a focused, patient-centred, goal-orientated plan of treatment and/or rehabilitation to avoid unnecessary admission or to facilitate early discharge.
- Participating in the provision of equipment and completion of manual handling plans following appropriate training.
