Job Overview
A fantastic opportunity has arisen within the Weight Management Department (WAMS) for a Patient Pathway Assistant, working Monday to Friday, 08:00–16:00 (37.5 hours per week, with one working from home day per week).
As Assistant Patient Pathway Coordinator you will be responsible for overseeing all aspects of the Weight Management and Assessment (WAMS) patient pathway, from referral through to treatment and on to follow up care.
You will be an integral part of the WAMS team, managing patients in accordance with key Government Access Targets and North Bristol Trust’s Patient Access Policy.
You will play a central role for the WAMS service and work closely with your colleagues across the wider multi-disciplinary team to move patients through the service pathway.
Your communication will extend to GPs, patients, the public and occasionally staff in other departments.
You will be processing the patients’ referrals and uploading documentation including their access plans to the hospital systems, namely Bluespier and Careflow. You will be responsible for booking patients’ appointments, recording patients’ clinical outcomes and encouraging timeliness within your wider team.
Main Duties of the Job
The Assistant Patient Pathway Coordinator role involves managing the patient pathway from referral to discharge. This looks like:
- Answering patient queries via telephone and email
- Booking clinical appointments using PAS systems (namely Careflow and Bluespier)
- Problem-solving proactively when managing patient flow (managing DNAs, cancellations, waiting list transfers)
- Liaising with clinicians to action outcomes for patients on the pathway
- Ensuring timeliness of clinic bookings to tackle our long waiting list
- Valuing constant and continuous communication within the immediate and wider team via email and in-person
- Centring compassionate care for patients in your communications with them
- Learning and adhering to our Standard Operating Procedures (SOPs)
- Adhering to important service timescales, to put patient safety and satisfaction at the heart of all you do
- Working closely with your admin colleagues in person and online as a team
- Actively seeking clarity and support from your colleagues to learn fast and strengthen competence quickly
- Holding yourself accountable for your mistakes, focusing on learning from them to enhance your service delivery

Person Specification
Education/Training/Qualifications
Essential criteria
- Minimum five GCSE/O’level qualifications OR equivalent - to include English Language and Mathematics
Desirable criteria
- NVQ Level 3 in Business Administration
Work Experience
Essential criteria
- Relevant health or social care experience at AFC Band 2, 3 or equivalent
- Experience in MS packages such as Word and Excel
- Previous experience in using hospital systems (PAS) such as Dictate IT, ICE, CRIS, Careflow, Bluespier
- Experience of dealing with data of a sensitive and confidential nature
- Experience of working under pressure
Desirable criteria
- Experience of working in a multi-disciplinary team
- Experience of working in a busy environment
