Employment, Justice and Health deliver integrated support to help individuals with complex barriers move towards and into work. We work with people affected by health conditions, justice involvement, or long-term unemployment, supporting them to improve their wellbeing, confidence, and employability. We provide tailored, person-centred support alongside access to specialist partners, and collaborate with employers to create inclusive job opportunities. Through this work, we aim to improve life chances and support sustained, positive outcomes for individuals and communities.
Driving license and access to a car are essential for this role due to travel requirements across county.

Key Responsibilities:
- Manage a caseload of participants, providing tailored support to help them achieve their employment goals.
- Conduct Work and Health Assessments both in person and online to identify employment support needs.
- Develop individual action plans to help participants overcome barriers and move towards sustainable employment.
- Source suitable job opportunities through employer engagement, local labour market knowledge and targeted job searches.
- Work collaboratively with NHS services, GP Practices, social prescribing teams and community partners to support participant outcomes.
- Deliver triage support and refer participants to appropriate specialist services where required.
- Support employers with workplace adjustments, return-to-work plans and job retention strategies.
- Maintain accurate records, data entry and case notes in line with programme requirements and organisational policies.

About You:
- Passionate about helping people improve their health, wellbeing and employment prospects.
- Experienced in supporting individuals facing complex barriers, including health conditions or disabilities.
- Comfortable building strong relationships with participants, employers and external stakeholders.
- Motivated by achieving positive outcomes and making a meaningful difference in people's lives.
- Confident working independently whilst contributing effectively within a multi-disciplinary team.
- Empathetic, approachable and able to build trust with a diverse range of individuals.
- Adaptable and able to manage a varied workload in a customer-focused environment.
- Punjabi, Urdu and/or Bengali language skills would be advantageous for this role.

Skills Required:
- Strong interpersonal and communication skills.
- Ability to manage a caseload and prioritise competing demands effectively.
- Experience delivering guidance, advice, coaching or person-centred support.
- Knowledge of the challenges faced by individuals with health conditions and other barriers to employment.
- Ability to build and maintain professional relationships with a range of partners and stakeholders.
- Good understanding of local support services and referral pathways.
- Accurate administration, record-keeping and data management skills.
- Proficient in using Microsoft Office applications and digital case management systems.
