Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

WorkWell is a pioneering service designed to bridge the gap between health and employment, helping people with physical and mental health conditions to move towards, stay in or return to work. As a WorkWell Coach, you'll empower individuals to overcome barriers, build confidence and achieve goals that matter to them.

This is more than a coaching role. You'll work alongside healthcare professionals, employment services, employers and community organisations as part of a multidisciplinary team committed to improving lives. Using a person-centred, strengths-based approach, you'll support participants to identify what is holding them back, develop personalised action plans and access the right services at the right time.

Every day brings variety, challenge and reward. You'll help people recognise their potential, take positive steps forward and achieve meaningful outcomes, while shaping a brand-new service that will make a lasting difference across the community.

As a WorkWell Coach, you'll provide tailored one-to-one support to a caseload of participants, delivering coaching face-to-face, online and by telephone. Through holistic assessments, you'll explore health, wellbeing, employment and social factors to understand each person's unique circumstances and co-produce practical, achievable plans that support their journey towards sustainable employment.

  • You'll provide ongoing coaching and mentoring, helping individuals develop confidence, resilience and work-readiness skills through activities such as goal setting, job searching, CV development, interview preparation and wellbeing support.
  • Working in a trauma-informed and strengths-based way, you'll also connect participants with specialist services and community resources, advocating on their behalf where needed.
  • You'll build positive relationships with employers and partner organisations, supporting conversations around workplace adjustments and return-to-work planning.
  • Alongside delivering high-quality, person-centred support, you'll manage your own caseload, maintain accurate records, monitor outcomes and contribute to the success and growth of this exciting new service.

Additional Working Pattern: Some evening and weekend work may be required.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles. Depending on the area you are applying to work in, this could include potential changes to your role, work base, reporting arrangements, or terms and conditions of employment. Any changes would be subject to consultation and managed sensitively and in line with relevant employment processes.
