We have an exciting opportunity for a Young Adults' Transitional Worker who has the skills and experience to provide assessment, advice, care-planned treatment, and ongoing support to young people experiencing serious or complex substance-related issues. A key element of the role is coordinating transitions, both into adult treatment pathways and into meaningful education, employment, or training opportunities.

Hours: Full-Time, 37.5hrs per week

Responsibilities

About the role

  • Strengthening coordination of multiple and simultaneous transitions to adult support (e.g., health, criminal justice, social care, welfare, and education).
  • Providing tailored, age-appropriate care & treatment for 18–25-year-olds within the Service.
  • To engage young people in a range of interventions, utilizing a young person-centred approach, empowering them to reduce risk, increase resilience, and lead a safe, happy, and healthy life.
  • Providing screening, assessment, psychosocial interventions, integrated care planning, post-treatment.
  • Identifying and responding effectively to potential safeguarding.
  • Reducing drug and alcohol-related harm to service users and the wider community.
  • Promoting carer, service user, and community.
  • Providing advocacy for access to partnership.
  • Working with service users to support positive, holistic outcomes in relation to their health and wellbeing, enabling them to lead safe, healthy, and purposeful lives: reducing risk and increasing resilience.

About you

  • Knowledge and experience of providing interventions for service users affected by substance use.
  • Be an excellent communicator, both verbal and written, with good IT skills.
  • Proactive and able to work on own initiative with excellent time management and prioritizing skills.
  • Have a good working knowledge of mental health interventions, services, and good practice.
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
  • Ability to manage change successfully in a way that prioritizes the needs of service users.
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute to and oversee the learning and development of others.

Salary Range (pro rata if part time)

  • CGL points 23 to 28 (£28,557.79 - £32,802.41)
  • ILW / OLW / Fringe: N/A - Outside London Weighting Area
