Role: Young Persons Practitioner
Salary: £26,006.84 per annum (pro-rata for part-time)
Location: Flintshire
Contract/Hours: Permanent, Part-time – 22.5 hours per week

A bit about the role
We are looking for a compassionate and motivated Young Persons Practitioner to deliver high-quality support to children, young people and their families. Working within a multi-agency environment, you will provide specialist interventions on a one-to-one, group and community basis, helping young people overcome challenges and achieve positive outcomes.
The role involves assessing needs, managing risk, coordinating support plans and acting as a lead practitioner where appropriate. You will ensure the voice of the child remains central to all aspects of your work while contributing to collaborative planning and decision-making with professionals across social care, education and health services.
What you do could make a difference
- Deliver high-quality, outcome-focused support to children, young people and families based on assessed need.
- Undertake assessments, develop support plans, review progress and manage risk effectively.
- Provide specialist interventions through one-to-one work, group programmes and community-based activities.
- Ensure children and young people remain at the centre of all interventions, with their views, wishes and feelings represented in meetings, plans and records.
- Attend and actively contribute to multi-agency meetings, reviews and professional discussions.
- Review and evaluate support provided, contributing to continuous service improvement.
- Undertake home and community visits in line with lone working policies and procedures.
Let’s talk about you
- Experience of working within a social care, education, health or community-based support environment.
- Ability to build positive, professional relationships with children, young people, families and partner agencies while maintaining appropriate boundaries.
- Great organisational skills, with the ability to prioritise a varied workload and manage competing demands.
- Demonstrable commitment to equality, diversity and inclusion, and experience of working within diverse communities.
- Knowledge and practical application of safeguarding procedures and child-centred practice.
- Ability and willingness to undertake home and site visits in accordance with lone working procedures.
- Working towards a Level 3 Diploma in Youth Work, Counselling or a related qualification.

About the Service
The Flintshire Families Project provides early intervention, prevention and whole-family support to children, young people and families across Flintshire. Working in partnership with Flintshire County Council's Early Help Hub and aligned with the Welsh Government's Families First programme, the service helps families access the right support at the right time.
Through the delivery of information, guidance, early years support and targeted interventions, the project works collaboratively with families and partner agencies to strengthen resilience, improve outcomes for children and young people, and prevent difficulties from escalating. Our approach is child-centred, outcome-focused and built on the belief that every family deserves the opportunity to thrive.
