The Role
To lead and deliver clinical pharmacy services across medical specialties at University Hospital Wishaw, including provision of pharmaceutical care, medicines advice and independent prescribing.
The post holder will be an integral member of the consultant-led multidisciplinary team and will contribute to both inpatient care and outpatient/ambulatory services, including management of patients within a post-MI LVSD clinic.
The role requires advanced clinical decision making, with responsibility for prescribing and deprescribing in complex medical patients, ensuring safe, effective and patient-centred use of medicines.
The Post Holder Will Lead On Development Of Clinical Pharmacy Services Within Medicine And Contribute To The Clinical Governance Agenda And Associated Systems For Medicines Use Within The Service. This Will Include But Not Be Limited To
- Provide expert pharmaceutical advice within medical services
- Act as an independent prescriber for complex patients, including clinic-based care
- Participate in consultant-led ward rounds and multidisciplinary team working
- Lead quality improvement initiatives and service development within medicine
- Support and develop pharmacy staff within the clinical team
- Deliver pharmacotherapy activities including medicines reconciliation, review and optimisation
- Contribute to education and training of pharmacy and wider MDT staff
- Contribute to rota including weekends and public holidays

There will be a focus on optimal use of medicines to ensure a patient-focused, efficient, progressive, resilient, safe and effective clinical pharmacy service within medicine.
In This Key Role, You Will
- To lead clinical pharmacy services for patients admitted to, or treated within, area of responsibility in accordance with the priorities set by the post holder, Hospital Pharmacy Manager, Lead Pharmacist Primary Care or Lead Pharmacist Advanced Clinical Services, and Clinical Director in order to ensure safe, effective and cost efficient use of medicines in these patients.
- To identify and prioritise clinical pharmacy services to the directorate/clinical specialty.
- To monitor clinical pharmacy services for specified patient population or directorate / clinical specialty in accordance with the objectives set by the post holder and Hospital Pharmacy Manager and Principal Clinical/Deputy Pharmacy Manager or Lead Pharmacist Primary Care or Lead Pharmacist Advanced Clinical Services.
- To demonstrate expert clinical knowledge with advanced level of clinical reasoning and judgement.
What You'll Bring
- Masters degree in Pharmacy.
- Registered to practice with the General Pharmaceutical Council.
- Continuing Professional Development as detailed by the General Pharmaceutical Council (mandatory requirement).
- Ability to monitor patient therapeutic regimens to ensure effectiveness and reduce toxicity.
