Discharge Facilitator (Fixed Term – 12 Months, Days Only)
About the Role
NHS Lothian is looking to recruit a Discharge Facilitator at Western General Hospital (Discharge Hub). This is a Fixed Term 12-month position, working Monday–Friday, days only.

Responsibilities While full details are available in the job description and person specification, the role will include:
- Acting as a discharge liaison specialist for patients in transition from hospital to home.
- Supporting multi-disciplinary teams in coordinating patient discharge to minimise avoidable delays.
- Adhering to care pathways expediting discharge, wherever possible.
- Assisting patients and families with post-discharge needs and signposting to appropriate services.
