Contract Type: Permanent
Salary: £25,877.80 - £28,453.69 per annum
Please note: The salary listed reflects the full earning potential for this role. Starting salaries depend on experience and progression within the band.

About the role
This is an exciting opportunity to play a key role in supporting the delivery of a vital service that helps people experiencing homelessness or at risk of homelessness to leave hospital safely and access the support they need to rebuild their lives.
As an Administrator, you'll provide a welcoming and professional experience for the people we support, colleagues and partner organisations. You'll deliver a wide range of administrative support, ensuring the service operates efficiently and effectively while maintaining high standards of accuracy and confidentiality.
Working closely with the wider team, you'll manage enquiries and referrals, coordinate appointments, maintain electronic records and support the day-to-day running of the service. You'll also work alongside colleagues and partner agencies to ensure information is shared appropriately, meetings are coordinated and administrative processes support the delivery of high-quality, person-centred services.

About the service
The Housing, Health & Wellbeing Hospital Discharge Support Service aims to prevent individuals aged 18 and over from being discharged from hospital into street homelessness, while supporting safe, timely and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support.
The service provides proactive, responsive and coordinated support through:
- Early identification of individuals who are homeless or at risk of homelessness.
- In-hospital assessment, advocacy and discharge coordination.
- Time-limited post-discharge support to help people sustain accommodation and reduce the risk of hospital readmission.
The service specifically supports individuals who:
- Are homeless or at risk of homelessness.
- Have support needs relating to drug and alcohol use.
- Are known to Adult Social Care services, including those with assessed care and support needs, those open to Adult Social Care teams, and those requiring coordinated discharge planning.
About you
We're looking for someone who is organised, approachable and committed to delivering excellent administrative support.
You'll have experience working in a busy administrative or customer-focused environment and be confident managing a varied workload while maintaining a high level of accuracy and professionalism.
You'll also have:
- Experience in an administrative or customer service role.
- Excellent organisational skills with the ability to prioritise competing tasks and meet deadlines.
- Experience using Microsoft Office and electronic record management systems.
- Strong written and verbal communication skills.
- Excellent attention to detail and the ability to maintain accurate, confidential records.
- The ability to work independently while contributing positively as part of a supportive team.
