Administrator Role – Integrated Transfer of Care Hub (ITOCH)
About the Role
The Administrator role will be an integral part of the Integrated Transfer of Care Hub (ITOCH), supporting with essential administrative functions aligned to the ITOCH operating model. This role is critical to fostering safe and effective discharges, operating across 7 days a week.
An Integrated Transfer of Care Hub (ITOCH) serves as a central point for coordinating discharges for individuals with new or increased health and social care needs, particularly those on discharge pathways 1, 2, and 3. This includes managing all complex discharges into intermediate care.
Important Notice: This role is not eligible for Skilled Worker visa sponsorship. Applicants must possess the right to work in the UK at the time of application, as the Trust is unable to support visa applications.
Key Responsibilities
- Be the first point of contact for enquiries related to the ITOCH service, IT, and Estates.
- Act with own initiative when addressing queries from:
- General public
- Staff
- Other agencies
- Families
- Patients
- Provide administrative support to ITOCH staff, including:
- Processing referrals
- Inputting data accurately
- Handling telephone queries
- Managing photocopying and general office duties
