Adult Social Worker - Mental Health
Liquid Personnel is currently recruiting for an experienced Social Worker to join its client’s Integrated Cluster Network Team based in Bridgend, Wales. This is an exciting opportunity to work within a multi‑disciplinary service that supports adults with long‑term, complex needs relating to physical health, mental health, and emotional well‑being. The team works with both younger and older adults, adopting a strengths‑based, person‑centred approach to ensure individuals are supported to live their lives with choice, independence, and control. Responsibilities Undertake strengths‑based, person‑centred assessments under the Social Services and Well‑being (Wales) Act 2014, identifying eligible care and support needs for adults and carers Develop, implement and review care and support plans that promote independence, well‑being and community inclusion Complete assessments relating to hospital admission, discharge planning, community living, and mental health needs Support individuals to access appropriate services, including domiciliary care, reablement, residential care, equipment, adaptations and direct payments Work with adults experiencing mental health difficulties, contributing to assessment, care planning and recovery‑focused intervention Complete or contribute to Care and Treatment Plans under the Mental Health (Wales) Measure 2010 Work in partnership with health professionals within multi‑disciplinary teams Where qualified, undertake duties as an Approved Mental Health Professional (AMHP) and contribute to Mental Health Act assessments Requirements Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW. Registration: Must be registered with Social Care Wales. Experience: 2 years post-qualified experience working in a statutory setting. Legislation Knowledge: Strong understanding of the Mental Health Act 1983, Mental Capacity Act 2005, Social Services and Well‑being (Wales) Act 2014, safeguarding procedures, and the Mental Health (Wales) Measure 2010. Skills: Excellent assessment, communication, and risk‑management skills, with the ability to work effectively in a fast‑paced multidisciplinary environment. Driving Licence: Full UK driving licence and access to a vehicle is often required for community visits. Why Liquid Personnel?
