This role has a starting salary of £31,659 per annum, based on a 36-hour working week. This is a full-time position but part-time applications are also welcomed.

We are hiring a Social Care Assistant to join our Connect to Community (C2C) team, our new front door service for Adult Social Care. This is an exciting opportunity for people who want to support residents, carers and families while helping to embed a modern, strengths-based approach to social care.

Your contractual base will either be Guildford Borough Council, Millmead House, Guildford but will be covering the geographical areas of Guildford, Waverley and Surrey Heath.

This is a hybrid role where the expectation is a minimum of 2 days per week in the office, as well as working in locations to meet the demands of the role as required.

About The Team

The Connect to Community (C2C) team, creates a seamless journey for residents. Our C2C Teams act as the first point of response, providing advice and information, urgent assessments, safeguarding, and support planning.

Our operating model is designed to cut down on unnecessary bureaucracy and increase meaningful time with people in their communities, focusing on what really matters: listening, building relationships and creating solutions that work.

About The Role

As a Social Care Assistant / Senior Social Care Assistant in Connect to Community, you will provide practical, frontline support to adults and carers at the first point of contact with Adult Social Care. In this role, you will gather relevant information and offer advice, guidance and appropriate signposting, while supporting strengths-based conversations that promote independence and wellbeing. You will assist carry out proportionate assessments with the right level of support and contribute to duty and intake activity, including responding to urgent enquiries.

You will play a key role in signposting residents to community, voluntary and universal services, supporting access to reablement, technology-enabled care and equipment, and identifying any safeguarding concerns so they can be escalated appropriately. The role also involves working collaboratively with colleagues and partner agencies to ensure coordinated and effective support.

Alongside this, you will manage a varied workload, maintain accurate and timely records, and consistently demonstrate a professional, person-centred approach in your work.

Your Application

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours:

  • The ability to empathise with, and show compassion towards, individuals in need of social care assistance.
  • Strong knowledge of the needs and challenges faced by adults requiring social care, and a commitment to supporting their well-being.
  • Proven ability to prioritise and manage multiple tasks effectively, ensuring timely and high-quality outcomes.
  • Proficiency in basic IT skills such as Microsoft Office, email and file management alongside a demonstrated willingness to learn and adapt to new software systems.
  • Excellent communication skills, with the ability to convey information clearly, concisely, and respectfully, both verbally and in writing.

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.
