Social Worker – Discharge Service
Salary: £43,633 per annum (36-hour working week)
About the Role
Working in a hospital setting is fast-paced and time-pressured. You’ll be part of the discharge planning process for patients and attend daily multidisciplinary meetings with professionals including Consultants, Occupational Therapists, Case Managers, and Nursing staff. This role offers a collaborative, supportive environment for those passionate about learning and growth.
We’re looking for a Social Worker to join our team at Royal Surrey Hospital, based in Guildford, covering Royal Surrey County Hospital and Milford Hospital. There may be an expectation of working at the Milford site, with a service running from:
- Weekdays: 8:00am – 8:00pm
- Weekends: 9:00am – 5:00pm
This role requires onsite presence with some hybrid working flexibility. Further details on working patterns can be discussed at interview.
###Your Responsibilities
- Conduct Care Act assessments
- Perform Mental Capacity Act assessments, including leading Best Interest Decisions
- Screen residents for Continuing Healthcare
- Carry out carers’ assessments
- Work on safeguarding cases and partner with NHS colleagues (including Discharge to Assess)
- Attend Multi-Disciplinary Team meetings across wards
- Support adults and their families/carers through discharge
You’ll also collaborate with:
- Adult Social Care teams (frontline and specialist)
- Community health services
- Charity and voluntary organisations

About the Team
The team operates across Royal Surrey County Hospital—one of five acute hospital sites in Surrey—and supports discharges from Milford Hospital’s community rehab beds. The focus? Getting patients back home safely using packages of care or short-term placements under the Discharge to Assessment framework.
You’ll be supported through:

Skills Expected
To be shortlisted, you must demonstrate the following in your application:
- Qualified Social Worker with current registration with Social Work England
- Completed an Assessed and Supported Year in Employment (ASYE, if qualified since 2012)
- In-depth knowledge of Care Act and Mental Capacity Act guidance, and their practical applications
- Experience in:
- Safeguarding enquiries
- Hospital/Community assessments
- Working with diverse clients and agencies
- Ability to mentor non-qualified staff and commit to ongoing professional development
- Willingness to work onsite each day
Surrey’s Context
Amidst Local Government Reorganisation (transitioning to unitary councils in April 2027), your role may transfer under the same terms. Activities reflect a commitment to community-led solutions and policy innovation.

- A valid UK driving licence
