Advanced Clinical Practitioner

Permanent Full-Time Role

Working 37.5 hours per week, based at Poppy Ward, Ellesmere Port Hospital

About the Role

The successful candidate will play a crucial role in enhancing clinical practice, research, and standards of care within the service. Key responsibilities include:

  • Assessing, treating, and directing patients to ensure a seamless care pathway.
  • Supporting the development of policies and guidelines to improve patient outcomes.
  • Collaborating on service development initiatives to create unified patient journeys.

Due to operational requirements, the post-holder may occasionally be called upon to work across other wards or departments within the Trust, particularly during periods of hospital pressure or major operational disruptions.

Service & Trust Context

The Countess of Chester Hospital NHS Foundation Trust operates across three key locations:

  • The Countess of Chester Hospital – 473 general and acute beds
  • Ellesmere Port Hospital – 60 beds (specialising in rehabilitation, intermediate, and outpatient care)
  • Tarporley War Memorial Hospital – Provides community services for the local population

With a headcount of 5,964 staff, the Trust serves a population of over 400,000 people across Chester, West Cheshire, Ellesmere Port, Neston, Deeside (Flintshire), and adjacent regions of North Wales.

The Trust operates as part of the Cheshire and Merseyside Integrated Care System and works closely with Betsi Cadwaladr University Health Board due to its proximity to the Welsh border.
