Advanced Clinical Practitioner (ACP) Home Visiting Service

The Role

Advanced practice is where clinical expertise, autonomy and decision-making come together.

Its where you assess complex presentations, manage uncertainty, and make timely decisions that have a direct impact on patient outcomes. Its varied, rewarding work that allows you to use your full clinical skillset while helping patients access the right care, at the right time.

At IC24, we're looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services.

As an experienced clinician, you'll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. You'll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes.

This role forms part of our East Sussex Home Visiting Service, supporting patients across the community. You'll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate.

We're looking for experienced clinicians who combine strong clinical expertise with excellent communication and decision-making skills.

You're likely someone who:

  • Enjoys working autonomously while remaining part of a wider multidisciplinary team
  • Is confident assessing and managing urgent and undifferentiated presentations
  • Has strong clinical reasoning and risk management skills
  • Communicates effectively with patients, carers and healthcare professionals
  • Enjoys delivering care within patients' homes and community settings

To be successful in this role, you'll need:

  • Professional registration with the NMC, HCPC or GPhC
  • Independent Prescriber qualification
  • Advanced assessment and minor illness qualification (Level 6 or 7)
  • Proven experience of autonomous clinical practice
  • Strong clinical decision-making skills and ability to manage complex presentations
  • Excellent communication and interpersonal skills
  • Commitment to continuous professional development and reflective practice
  • Evidence of service improvement and innovation

  • Additional East Sussex operational bases as required

2 Positions available at 37.5 hours per week
