The Role
The Advanced Clinical Practitioner (ACP) within the Intermediate Care (IMC) service provides expert, autonomous clinical care to patients with complex and acute rehabilitation needs with long term medical needs. Working as a core member of the multidisciplinary team (MDT), the ACP delivers advanced assessment, diagnosis, treatment, and ongoing management across the IMC pathway to support safe, effective, and patient-centred care. The ACP will support safe discharges for the patients.
The ACP works collaboratively with consultants, nursing staff, therapists, pharmacists, and wider allied health professionals to ensure coordinated decision-making and continuity of care. The role involves leadership in clinical practice, contribution to service development, education, and quality improvement, and acting as a senior clinical resource within the team.
