Job Introduction

Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACP’s, ANP’s & APNP’s to join our workforce with opportunities within Primary Care, Out Of Hours, Alder Hey Hospital & Community Care. We employ salaried and bank workers, so offer the flexibility to fit around you.

Salary: £64,876.00 - £66,711.00 per annum (pro rata) or £33.18 per hour for Bank staff

Department: Out of Hours, Primary Care & Alder Hey

Location: Various locations within Merseyside, Sefton & Halton; including Wavertree Head Office

Shifts: Full time/part time and bank positions available (depending on department)

What will I be doing?

Out-of-hours (OOH): Onsite (with remote working available) - Weekdays after 6pm & Weekends

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
  • Work with patients to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.

Primary Care: Onsite in our GP practices - 8am - 6pm

  • Undertake comprehensive health assessments for adults and paediatric patients, applying advanced clinical judgement and holistic care planning.
  • Assess, diagnose, plan, implement and evaluate treatment and interventions for undifferentiated and undiagnosed conditions.
  • Perform clinical examinations and interpret findings from both physiological and psychological perspectives.
  • Manage a clinical caseload with a high degree of autonomy, making safe and timely decisions within agreed governance frameworks.
  • Contribute to a culture of patient safety, quality improvement and effective multidisciplinary working.
  • Maintain professional competence and development in line with regulatory requirements, contributing positively as a senior clinician within the practice team.

Alder Hey: Onsite - 9am - 9pm

  • Deliver quality individualised care to children and young people with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Within your professional boundaries you will assess, diagnose and prescribe where appropriate, providing advice and care for children and young people presenting with undiagnosed and undifferentiated illnesses.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
  • You will practice autonomously whilst demonstrating a child and young person-centred approach to meet PC24 patient health care needs.
  • The position will require the ability to establish and maintain excellent professional working relationships with hospital, primary care and wider stakeholder colleagues.

What do I need?

  • Full NMC Registration with no restrictions
  • MSc in Advanced Clinical Practice is desirable; candidates who are working towards this qualification or who have relevant experience are encouraged to apply
  • Non-Medical Prescriber – V300
  • Clinical Examination and Diagnostics Level 7
  • Experience of working autonomously
  • Experience of working in urgent care and/or primary care
