Role
An excellent opportunity has arisen for an Advanced Clinical Practitioner to join our well-established Frailty/Care of Elderly Team. The role will require the successful clinician to work within the four pillars of Advanced Practice which includes:
- Clinical management of patients
- Lead on service developments and change
- Provide education, mentorship, and support to a wide multi-professional audience
- Demonstrate understanding and use of evidence-based practice and involvement in research (QIP/Audit)
Welsh Skills
This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. - Provide clinical care/management to groups of patients admitted to the Frailty/Care of Elderly service
- Lead on service development
- Provide leadership to MDT members
- Education: deliver educational packages to a multi-professional audience. Maintain own level of education
- Research: be involved in relevant research/audit/SE/QiP's
