Job Summary
We are seeking experienced and highly motivated full time and part time Advanced Clinical Practitioners to join our dynamic and inclusive multi-disciplinary team within Taurus Healthcare Clinical Services.
This is an exciting opportunity to work at the forefront of integrated care, delivering high-quality, patient-centred services across urgent and primary care pathways.
As an ACP, you will work autonomously and collaboratively with multi-disciplinary teams to assess, diagnose, treat, and manage patients presenting with a wide range of acute and chronic conditions
Due to current licence limitations, visa sponsorship is not available for this position. Applicants must already hold the right to work in the UK.

Main duties of the job
- Provide advanced clinical assessment, diagnosis and treatment for patients of all ages.
- Manage undifferentiated and complex presentations in an urgent care setting
- Prescribe medications independently and implement treatment plans
- Request and interpret diagnostic investigations
- Work collaboratively with GPs, nurses and the operational team
- Support service development and contribute to quality improvement initiatives
- Provide clinical leadership, mentorship, and education to junior staff

Job Description
JOB PURPOSE:
The Advanced Clinical Practitioner (ACP) is a senior autonomous clinician who delivers high quality, evidence based care across Taurus Healthcare's integrated primary care and urgent care services, supporting the organisations vision of accessible, 24/7 general practice.
The post holder will assess, diagnose, treat and manage patients with undifferentiated, acute and complex conditions, while contributing to service development, clinical leadership, education and quality improvement across the system.
Main Responsibilities
Clinical Practice
- Practice as an independent and autonomous clinician, managing patients with undifferentiated, acute and chronic conditions across face to face, home visiting, remote and digital consultation models
- Undertake comprehensive clinical assessment, diagnosis and management, demonstrating advanced clinical reasoning, risk assessment and safety netting
- Initiate, interpret and act on appropriate investigations and referrals in line with local and national pathways
- Prescribe and manage medications safely and effectively in accordance with national guidance, local formularies and EMIS decision support tools
- Contribute to flow, continuity and demand management across integrated services, including prevention of unnecessary hospital admissions
- Respond confidently to complex, urgent and emergency presentations, initiating immediate care as required
- Safeguard children, young people and adults, recognising risk, acting within statutory frameworks and escalating concerns appropriately
Integrated & Multidisciplinary Working
- Work collaboratively with GPs, nurses, paramedics, pharmacists, operational teams and external partners to deliver seamless, patient-centred care
- Support system wide working by ensuring patients receive the right care, in the right place, at the right time
- Contribute to the development of new models of care that enhance access, quality and patient experience

Leadership & Service Development
- Act as a senior clinical decision maker within the multidisciplinary team
- Provide visible clinical leadership, supporting safe practice, innovation and continuous improvement
- Contribute to service redesign, pathway development and quality assurance activity
- Support the investigation and learning from clinical incidents, audits and significant events
- Promote a positive learning culture, role modelling professionalism and reflective practice
Person Specification
Personal Qualities or attributes
- Essential
- Commitment to integrated, patient-centred care and service innovation
Qualifications
- Essential
- Registered Nurse (NMC) or Paramedic (HCPC).
- MSc Advanced Clinical Practice (or equivalent credentialed ACP qualification) or be working towards completion of an approved MSc in Advanced Clinical Practice.
- Registered Non-Medical Independent Prescriber.
Skills
- Essential
- Proven ability to work autonomously while recognising professional limits
- Excellent communication, clinical reasoning and decision-making skills
- Driver with clean licence
Experience

- Minimum 5 years post-registration experience, including significant autonomous practice.
- Demonstrable experience managing undifferentiated and complex clinical presentations.

- Experience working across primary care and urgent care settings.
- Experience of EMIS and remote consultation platforms.
- Leadership or service development experience.
- Experience of supporting education, supervision or mentoring
