The Role

An exciting opportunity has arisen for an Advanced Diabetes Nurse Specialist to work within the District Nursing Service within Renfrewshire HSCP. It is essential that applicants are on the NMC Register as an Adult Nurse and have significant experience in Diabetes care with a portfolio of evidence demonstrating achievement of relevant specialist diabetes competencies necessary for the role. This includes an appropriate post graduate diploma in Diabetes at masters level (or 120 credits equivalent) and a non-medical prescribing qualification.

The Advanced Diabetes Specialist Nurse works in partnership with the wider multi-disciplinary team and across the acute and community interface to deliver holistic, individualised programmes of care to patients with diabetes who are referred by either primary or secondary care.

The post holder will work in conjunction with the referrer to provide highly specialist advice in order to optimise diabetes symptom control. They are required to undertake highly skilled assessment and clinical interventions at various stages for individuals living with this chronic disease process.

The post holder will have comprehensive, highly specialist knowledge within the field of diabetes and act autonomously in the assessment, planning and implementation of treatment plans for patients with a diagnosis of diabetes.

They will have overall responsibility for managing complex caseloads whilst delegating patients to the wider DN team. The post holder will be responsible for the design and delivery of specialist diabetes education and support to adults with diabetes, their families and carers and the wider MDT team across the Primary and secondary care interface. The post holder will act as a resource for expert advice and training for nursing staff, medical staff, other health care professionals and other agencies.

The post holder will lead and contribute to issues relating to diabetes care locally and nationally where appropriate The post holder is responsible for the delivery of the highest standards of patient centred care to patients and their relatives through the development and management of staff within their remit. The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience and will lead and enable the team to provide a high quality service.

All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:

  • Location: based in Renfrewshire HSCP
  • Contract Type: Permanent
  • Working Pattern: Full-time position (36 position)
  • Shift pattern: Monday-Friday 8.30am-4.30pm excluding Public Holidays

Key Responsibilities

  • Assess, plan and develop individualised care plans for patients who have diabetes and their families/carers in particular for those who require complex diabetes management.
  • Undertake high level clinical interventions at various stages of life for individuals living with diabetes; including Insulin initiation, insulin dose adjustment, GLP-1 initiation and supporting development of the skills necessary for self-management.
  • Provide Tele-Health consultations and advice in response to patient enquiry and need, enabling direct access to specialist advice to enhance self-management and minimise risk for patients at times of crisis or during periods of illness when risk of hospital admission/re-admission is greatest.
  • Deliver patient care based on principles of best practice and patient self-care, challenging and improving current practice as appropriate; ensuring practice is evidenced and research based.
  • Support patients through difficult life experiences that may affect their diabetes control.
  • Enable patients to set realistic targets for glycaemic control using a person-centred individualised approach optimising glycaemic control with life-style advice that meets their specific needs.
  • Support for patients having difficulty, adjusting to and living with their diabetes.
  • Provide management and support to those who have diabetic complications in order to reduce progression.
  • Promote collaborative working relationships and effective communications between primary and secondary care by regular engagement and attendance at MDT meetings to ensure a high standard of coordinated patient care.
  • Co-ordinate and participate in the mentorship, supervision and clinical education of learners and junior staff.

Knowledge, Training, Qualifications and/or Experience Required to Do the Job

  • Registered Nurse (Adult) on NMC Register with significant experience in Diabetes care with a portfolio of evidence demonstrating achievement of level 7 core Advanced CNS competencies and specialist diabetes competencies necessary for the role.
  • This should include:
    • An appropriate post graduate diploma (masters level) (or 120 credits equivalent)
    • Workplace based assessments, practice related feedback, and reflective accounts
  • Evidence of Continuing Professional Development.
  • Competent in the use of technology e.g. IT, PCs and software.
  • Highly effective communication skills, verbal and written.
  • Highly developed problem solving skills.
  • Non-medical prescriber (V300).
  • Ability to respond to the demands of the post with flexible working as required.
  • Ability to lead and manage a team.
  • Willing to undertake further training
