Advanced Nurse Practitioner – Hospital at Home Service

Our services include acute hospital, primary, community, and mental health care for a population of over 1.15 million people, extending to 2.2 million when regional and national services are included.


About the Role

We are seeking motivated Advanced Nurse Practitioners to join the emerging Hospital at Home Service across Renfrewshire and East Renfrewshire HSCP Health and Social Care Partnership.

This frailty-focused virtual ward model enables the delivery of acute-level care in patients’ own homes or homely environments, working in close collaboration with acute and community services.

You must hold a current NMC UK PIN to apply.

All other roles will be assessed for sponsorship eligibility under current Home Office regulations. NHSGGC can only issue a Certificate of Sponsorship (CoS) when compliance is assured.

Key Details

  • Location: Based at Royal Hospital Alexandra (Labs Building, Room 20) whilst supporting a virtual ward (up to 18 beds).
  • Contract: Permanent, full-time (36 hours per week).
  • Working Pattern: 8am–8pm across 5 shifts over 7 days, including public holidays.

  • Collaborate with acute front-door services (Emergency Department, Medical Assessment Unit, Acute Medical Unit, Older Adults Assessment Unit) to select, assess, and admit suitable patients to the virtual ward.
  • Conduct advanced clinical assessments, diagnoses, and decision-making alongside medical officers to design and implement personalised care plans.
  • Deliver hospital-level diagnostics and interventions at the patient’s home, adhering to standard operating procedures.
  • Provide ongoing clinical reviews, patient monitoring, and contribute to multidisciplinary decision-making.
  • Lead discharge planning for safe transitions to further community support, as appropriate.

Qualifications, Experience & Competencies

Essential

  • Qualified Advanced Nurse Practitioner (ANP) (not suitable for trainees).
  • Proven expertise in advanced clinical assessments, diagnoses, and decision-making.
  • Experience in acute or community settings at an advanced practice level.
  • Ability to work autonomously while contributing effectively to a multidisciplinary team.
  • Knowledge of frailty pathways, hospital-at-home models, or virtual wards (desirable).
  • Proficiency in delivering hospital-grade diagnostics or interventions in the community.
  • Strong communication and collaborative skills.
  • Full UK driving licence required for home visits.
