Advanced Nurse Practitioner/Advanced Clinical Practitioner, Community

Advanced Nurse Practitioner – Dwyfor Community Resource Team (Fixed Term/Secondment)

Job Overview

This is a fixed-term/secondment position until 29/02/2028 due to service requirements. Applicants must obtain permission from their current line manager prior to applying.

We are seeking an experienced and innovative Advanced Nurse Practitioner to join the Dwyfor Community Resource Team (funded by the Dwyfor Cluster). This role will support housebound patients across Dwyfor/Gogledd Meirionnydd, delivering:

  • Rapid response care for vulnerable patients
  • Planned management of chronic and acutely unwell patients with complex multiple comorbidities
  • Frailty assessment and initiation of Comprehensive Geriatric Assessment (CGA) and Anticipatory Advanced Care Planning

The role will promote excellence in nursing, focusing on high standards of care, monitoring, and evaluation.


Main Duties and Key Responsibilities

Clinical Practice & Service Delivery

  • Act as an autonomous Advanced Practitioner with an extended scope of practice, delivering specialist assessment and treatment.
  • Apply evidence-based principles and current best clinical practice for patient interventions.
  • Assess, plan, implement, and evaluate treatments using advanced clinical reasoning and specialist skills.
  • Ensure clinical practice is rooted in research and evidence, conducting audits and research where appropriate.
  • Contribute to quality improvement initiatives and clinical research projects to enhance patient outcomes.

Detailed Job Requirements & Person Specification

Qualifications (Essential)

  • BSc or Grad Dip in a relevant nursing/ex Allied Health Professional field
  • Master-level education in Advanced Clinical Practice (or equivalent)
  • Portfolio demonstrating Continuous Professional Development (CPD) and post-registration activity
  • Independent prescribing qualification

Experience (Essential)

  • Extensive clinical experience in a specialised nursing/Advanced Practitioner role.
  • Service development/project management experience.
  • Protocol/development pathway expertise.
  • Multidisciplinary Team (MDT) collaboration experience.
  • Proven track record in clinical education & mentoring.

(Desirable: Formal Research qualifications )

Skills (Essential)

  • Advanced clinical reasoning and analytical problem-solving skills.
  • Strong ability to prioritise personal and team workload.
  • Critical evaluation of research/evidence and evidence-based practice implementation.
  • Exceptional communication skills (oral and written).
  • Advanced negotiation, influence, and team leadership abilities.
  • Strong time management and organisational skills.
  • Awareness of health and safety/risk management principles.
  • Knowledge of clinical governance frameworks.

(Desirable: Ability to speak Welsh )

Knowledge (Essential)

  • Evidence of recent Masters-level CPD in the relevant clinical field.
  • Completed clinical postgraduate training/education.
  • Strong understanding of professional ethics/code of practice and their application in daily practice.
