Job Overview

Are you a pharmacist who is keen to make a meaningful difference to service provision, patient experience and outcomes in diabetes in the West Integrated Health Community (IHC)?

We're looking for a dynamic, forward-thinking pharmacist with a keen interest in diabetes to join our expanding integrated pharmacy team.

We will welcome you as a member of our friendly and supportive pharmacy teams in Ysbyty Gwynedd and the IHC West primary care team, and you will work with us to achieve our goal to deliver seamless, patient-centred pharmaceutical care that maximises the health and wellbeing of the people of North West Wales.

You will be supported in your credentialing journey and will be encouraged to further develop your clinical skills at advanced clinical practice level. Our established links with Welsh University will provide opportunities to get involved in teaching and mentoring undergraduate pharmacy students.

Whether you are a pharmacist with previous diabetes background or an experienced clinical pharmacist ready to step into a specialist clinical role we would love to hear from you.

Main Duties of the Job

  • Work directly alongside consultants and specialist nurses, holding your own clinic caseload as an independent prescriber
  • Practise as an independent prescriber in primary care clinics, pre-op clinics and acute inpatient wards delivering expert pharmaceutical care resulting in optimal treatment outcomes with a patient centred approach
  • Lead on safe, cost-effective prescribing across the specialty, including monitoring drug usage, expenditure, and initiatives in line with the value and sustainability agenda
  • Developing and implementing prescribing guidelines, policies, and protocols
  • Representing pharmacy at diabetes MDT and governance meetings, and contributing to regional and national specialty networks.
  • Acting as a clinical role model supporting, mentoring, and educating undergraduate pharmacy students, trainee pharmacists, rotational pharmacists, and other healthcare professionals.
  • Driving service development and innovation, with the opportunity to participate in research/ audit and share learning through publication and presentation at conferences.
  • Provide core pharmacy services in the dispensary and on inpatient wards on a sessional basis including weekend working and on-call

Qualifications

Essential criteria

  • MPharm degree or equivalent
  • Registered with the General Pharmaceutical Council.
  • Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications
  • Non-medical prescribing qualification - or working towards

Desirable criteria

  • Membership of relevant professional association
  • Accredited or eligible to be accredited as a Higher Education Institute Clinical Pharmacy diploma/MSc tutor
  • Training qualification Leadership and/or management qualification

Experience

Essential criteria

  • Must be able to demonstrate suitable level of experience working in the managed sector and/or community pharmacy

Desirable criteria

  • Experience in relevant clinical area
