Advanced Pharmacist – Medicines Management (Ceredigion & Pembrokeshire)
(Whole Time Equivalent, with flexibility for part-time or split roles between localities)
About the Role
Hywel Dda University Health Board is seeking an Advanced Pharmacist (or two candidates, one for each locality, with a split role) to support the Ceredigion and Pembrokeshire Medicines Management team.
The role offers flexibility—working from Aberaeron (Ceredigion) and Haverfordwest (Pembrokeshire) base locations, with a particular need for coverage in the Ceredigion area. Applications are particularly welcome from candidates with an interest in or willingness to upskill in diabetes.

As an Advanced Pharmacist, you will work across the four pillars of advanced practice, supporting primary care via independent prescribing, deprescribing, and medicines safety.
Key Responsibilities
You will be responsible for:
Clinical & Prescribing Leadership
- Provide expert clinical pharmacy services in primary care, demonstrating advanced practice across:
- Prescribing
- Medicines management
- Patient education & support
- Advanced nurse prescribing (if applicable)
- Offer specialist advice to the multi-disciplinary team (MDT) in primary care, including GP clusters, Prescribing Leads, and secondary care interfacing.
- Deliver prescribing workshops, Annual Prescribing Visits, and cluster training, using negotiation skills to drive changes in prescribing patterns—even where considerations may be contentious.
- Monitor prescribing trends, congregate data, and manage budgets to ensure cost-effective and ethical medicines use.
Medicines Optimisation
- Implement strategies to reduce unwarranted variation while maintaining high patient safety.
- Utilise prescribing analytics to assess financial impact and clinical benefits.
- Champion holistic, patient-centred care with a focus on:
- Appropriate deprescribing
- Cost-savings through improved steroid rationing and generic switching
- Supporting the full titration lifecycle for medications

Hybrid Working Model
The role operates in a flexible, hybrid model, requiring work:
- From base offices (Aberaeron & Haverfordwest)
- In primary care settings (GP practices)
- At meetings across Hywel Dda localities
Person Specification (Core & Desirable Criteria)
Essential Requirements
- Be a registered pharmacist with the GPhC
- Hold Independent Prescriber qualification (V150 or equivalent)
- Experience of working in primary or community care
- Strong knowledge of medicines optimisation, cost-effective prescribing, and Wales Stratified Care
Desirable
- Specialisation or interest in diabetes, mental health, or long-term conditions prescribing
- Experience of working in a collaborative MDT involving nurses and physicians
- Familiarity with data analytics, prescribing budgets, and financial governance
- Bilingual Welsh/English skills (an advantage, as the Health Board operates a bilingual policy)
