Advanced Practitioner – Adult Care Services
Starting Salary: £48,226 progressing to £53,460 per annum Hours: Full Time (37/week) Location: Various (Hertfordshire) Contract Type: Permanent (except as noted) Directorate: Adult Care Services
About the Team

We are seeking experienced and motivated Advanced Practitioners to join our Adult Social Care teams. This role offers an opportunity to develop your career while supporting adults through strength-based practice, promoting independence, and fostering collaborative care within a supportive environment.
Our aim: To deliver high-quality, person-centred care and make a meaningful impact on the wellbeing of residents across Hertfordshire.
Key Teams & Specialisations
1. Continuing Healthcare Team (CHC)
Location: Hemel Hempstead Hours: 37/week | Fixed Term (until April 2027) Role Focus: Supporting NHS Continuing Healthcare processes with social care and health colleagues. Responsibilities include: • Coordinating complex assessments • Gathering multidisciplinary information • Ensuring timely progression under CHC requirements • Collaborating to deliver high-quality, person-centred practice
Key Skills Required: Organisational expertise, strong communication (e.g., managing difficult conversations), negotiation skills, and a deep understanding of CHC processes.
2. Adult Disability Team (East)
Location: Farnham House, Stevenage Hours: 37/week Role Focus: Supporting adults with learning or physical disabilities. Core features of the role: • Person-centred practice via Connected Lives framework • Managing complex cases and providing supervision • Ensuring statutory duties (safeguarding, assessment, care management) • Developing and mentoring existing/future team members
Requirements: Ability to handle intricate cases, commitment to empowerment, and experience in skill development via mentoring.
3. Older People’s Teams
(a) Prevention of Admissions Team
Location: Apsley, Hemel Hempstead Hours: 37/week | Permanent Role: Operates under the Care Act 2014 to safeguard, assess, plan support for adults with long-term needs. (b) East & North Herts Older Peoples Service
Location: Welwyn, Hatfield & Broxbourne Hours: 37/week | Permanent Role Focus: Supporting individuals to stay in their own homes and maintain independence. Tasks: • Demonstrable expertise in complex case management & safeguarding • Person-centred, empowering approaches • Mentoring/team leadership and community networking

(c) Mental Health Inpatient Unit
Location: Stevenage (with travel across inpatient units) Hours: 37/week | Fixed Term (until Aug 2027) Role: Ensures holistic, person-centred care for individuals with dementia in a mental health inpatient setting. Key responsibilities: • Completing social care assessments (in line with Care Act 2014) • Collaborating on care/discharge planning with clinical teams • Advocating for patient rights and preferences • Multidisciplinary multidisciplinary liaison to promote continuity of care • Support for safeguarding processes
Note: Travel required for weekly site visits and attendance at multidisciplinary meetings.
About the Role
Core Responsibilities
Advanced Practitioners in Adult Social Care provide expert leadership in key areas, including:
✅ Practice Development • Lead peer training & skill development in compliance with The Care Act 2014 • Supervise staff to embed Connected Lives principles (people-centred, strength-based, goal-orientated care)
✅ Statutory Duties • Conduct safeguarding, assessment, and care management • Deliver innovative, creative community solutions • Develop multi-agency partnerships (health professionals, providers, volunteers, families/carers) to advance collective outcomes
✅ Quality Assurance • Maintain all statutory requirements to high standards • Support equitable access to resources based on individual priorities
✅ Organisation & Collaboration • For the Mental Health Inpatient Unit role: work in interdisciplinary collaboration to design comprehensive discharge pathways • Draft accurate care plans and support discharge processes collaboratively with inpatient units
Stakeholders: Includes residents, families/carers, frontline staff, clinical teams, and external providers.
About You (Qualifications & Requirements)
Essential Qualifications & Experience
• Qualified Social Worker with at least 2 years’ post-qualifying experience • Demonstrable expertise in Adult Social Care, specifically: – Safeeguarding practice – Deep understanding of Care Act 2014, Mental Capacity Act 2005 – Experience navigating safeguarding legislation, notifications, reporting
Core Competencies & Expertise
• Specialist experience in one of these areas:
- Moving and Handling Practitioner
- Practice Educator
- Approved Mental Health Practitioner
- Best Interest Assessor OR commitment to undertake training in one of these roles within the first year.
• Advanced skills required: – Managing competing priorities and legislative compliance – Management potential, evidenced by supervisory experience or proven mentorship – Cross-sector negotiation (statutory, voluntary, and independent sectors) – Leadership in complex case management
