About the Role

The team specialises in supporting individuals within the Learning Disability and/or Autism Programme, helping to discharge people from long-term hospital placements who require specialised residential care. This includes those subject to the Mental Health Act 1983 (MHA), including Forensic Sections (MHA 37/41) and Ministry of Justice requirements.

You will work closely with commissioners, healthcare colleagues, and the Local Integrated Care Board (ICB) Learning Disability & Autism Programme to:

  • Prevent hospital admissions
  • Support community placements in crisis

Key Responsibilities

  • Apply expertise in:
    • Care Act 2014
    • Mental Capacity Act 2005
    • Mental Health Act 1983
  • Operate in a challenging but supportive environment with frequent collaboration across external agencies.
  • Demonstrate strong leadership, supervising social workers in the team.
  • Work innovatively with adults requiring complex care and support.
  • Embrace strengths-based practice, independence, and wellbeing while managing risks safely.
  • Provide and receive engaging opportunities for CPD through regular formal and informal supervision.

Requirements

  • Qualified Social Worker with additional expertise:
    • **Practice Educator_
    • **Best Interest Assessor (BIA), or Approved Mental Health Professional (AMHP)
  • Driving: Must possess and maintain access to a vehicle (or eligible for taxi services if mobility restricted).
  • Social Work England Registration
  • Excellent communication, negotiation, organisational, and IT skills in a hybrid setting.
