Job Description
Salary range: £53,172 - £55,323 p.a. plus fantastic benefits
Contract: Permanent
Hours of work: 36 hours per week

The Role: Advanced Social Worker (LIFE Team)
We are looking for an experienced and motivated Advanced Social Worker to join our LIFE (Living Independently for Everyone) Team.
This is a fast-paced role focused on hospital discharge, working within the Discharge to Assess (D2A) model. You will support individuals to return home safely, delivering strength-based assessments that promote independence and recovery.
Key Responsibilities
- Lead and deliver high-quality strength-based assessments in the community
- Support hospital discharge pathways through short-term, outcome-focused interventions
- Provide supervision, mentoring, and guidance to Social Workers and Health & Wellbeing Assessors
- Participate in the Duty Senior rota, allocating work and offering professional advice
- Work within a multi-disciplinary, integrated team to deliver person-centred care
- Contribute to service development and continuous improvement

About You
You will be:
- A qualified Social Worker with post-qualifying experience
- Confident in supervision and supporting staff development
- Experienced in adult social care practice, ideally including discharge or reablement
- Skilled in strength-based practice and person-centred approaches
- Able to work flexibly in a fast-paced and evolving environment
- A collaborative team player with professionalism, creativity, and drive
