Are you experienced, skilled, and passionate about making a meaningful difference? We're seeking a dedicated Aftercare Recovery Worker to join our West Kent Team.

Fixed Term Contact of 12 months from start date
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the Role
- Support the service user to connect with activities, support to sustain recovery and reach their goals set out in their aftercare plan e.g. training, employment support.
- Facilitate post-treatment groups across West Kent in community settings, outside of CGL hubs, to support individuals in progressing their recovery journey independently of services.
- Provide service users with information on options available to them within the service and in their communities.
- Collaborate with the Tier 4 Worker, Locality Leads, and Partnership Lead to engage partner agencies that service users can access for support following treatment.
- Build positive relationships with aftercare providers to ensure recovery support is available to support post-Rehab early abstinence i.e., Build on Belief (BOB), Reach out & Recover (ROAR), Fellowship Groups, Aspire2be, SMART.
- Work collaboratively with the Service User Inclusion Worker to understand post treatment needs, ensuring the voices and preferences of service users are heard and reflected.
- Working with Peer Mentors/Volunteers to provide Recovery Check-Ins.

What we are looking for
- Excellent knowledge of inpatient detox and residential services.
- Experience of working within multidisciplinary teams and with fostering positive relationships with partner agencies and external providers.
- Experience of working with people at all stages of their treatment journey and an in-depth knowledge of the importance that groupwork and aftercare support offers.
- Excellent written and verbal communication skills and the ability to provide effective advocacy when needed.
