Location: Covering Northampton Primary Care Network
Northampton/Hybrid

Rate of pay: £18,295.68 per annum (FTE £28,205.84)

Hours of work: 24 hours per week

About the Age Well Team

Age Well helps older people stay healthy, independent, and living well in their communities. Each Age Well team is based within a group of GP surgeries that work together as a Primary Care Network (PCN). Every PCN has a Lead Frailty GP and an Agewell Team made up from a partnership of organisations including Age UK Northamptonshire, Northamptonshire Carers, the Alzheimer’s Society, Northamptonshire Healthcare Foundation Trust (NHFT), and Adult Social Care teams from both North and West Northamptonshire Councils.

About the role

As an Age Well Coordinator for Northampton you’ll be part of this multi agency team, bringing together expertise from health, care and the voluntary sector. You will represent Age UK Northamptonshire, ensuring our warm, person centred approach is reflected in every interaction and support plan.

You’ll work closely with older adults, their families and carers to understand what truly matters to them. By coordinating personalised, community based support, you’ll help reduce loneliness, prevent decline, enhance wellbeing and connect people to activities, services and networks that help them thrive.

What you’ll be doing

Your days will be varied and rewarding. You will be visiting people at home, completing holistic assessments, making onward referrals, providing some practical support, working with GP practices, and community teams, contributing to multi-agency case discussions and in patient home meetings, and helping to shape age-friendly initiatives across local neighbourhoods. You’ll draw on your local knowledge, problem solving abilities, and genuine care for people to ensure older residents receive timely, holistic support.
