Assertive Outreach Team – Mental Health Nurse, Social Worker or Occupational Therapist

Join our Assertive Outreach Team and play a vital role in supporting individuals who may struggle to engage with traditional mental health services.

Based in Yeovil, you’ll work as part of a skilled multidisciplinary team, delivering person-centred care while fostering long-term positive outcomes for service users.

Key Responsibilities
- Provide person-centred, autonomous care to individuals with complex needs
- Work closely with a supportive, experienced team across community, inpatient, and voluntary services
- Develop leadership skills and contribute to service improvement initiatives
- Mentor junior staff and students, fostering professional growth

Requirements
- Qualified Mental Health Nurse, Social Worker, or Occupational Therapist
- Strong passion for mental health advocacy and community-based care
- Commitment to long-term engagement with vulnerable individuals
- Ability to work autonomously while collaborating effectively in a team
