Assessment & Review Officer – Adult Social Care (Permanent Post)
Location: North East Somerset Locality Team, Bath & North East Somerset Council
About the Role
Our mission: to improve people’s lives by empowering them to live independently, healthily, and happily within their community.
As part of our expanding Adult Social Care workforce, we are seeking a permanent Assessment and Review Officer to work closely with Social Workers, applying strengths-based approaches to support statutory duties in a person-centred, proportionate, and enabling way.
Key Responsibilities
- Conduct multi-agency assessments, using a strengths-based model to understand and address individuals’ needs.
- Develop, implement, and review care and support plans to ensure they align with shared/best outcomes.
- Collaborate with customers, carers, and stakeholders across agencies to ensure seamless coordination.
- Support Social Work Teams in delivering statutory duties under the Care Act 2014, including:
- Care Act (2014) review processes
- Carers’ assessments
- Providing in-person support at drop-in sessions for residents requiring care and assistance.

Requirements
- Strong commitment to person-centred practice and a willingness to embed strengths-based approaches.
- Proactive, highly motivated individuals who thrive in multi-disciplinary team environments.
- Excellent communication skills (written and verbal) to effectively engage with diverse stakeholders.
- Comfortable working with vulnerable individuals and a keen understanding of social care principles (at least on a basic level).
- A desire to continuously develop through structured supervision and learning opportunities.
