Vacancy Introduction

As an Assistant Case Manager based within the Acute Trust (St James’ University Hospital, Leeds General Infirmary, & Chapel Allerton Hospital) you will utilise your community knowledge and skills to ensure patients are supported onto the most appropriate discharge pathways, working closely with Adult Social Care colleagues and ward-based staff to facilitate timely safe discharge planning.

You will be aligned to wards to build relationships with patients and ward staff to support with discharge planning at an early stage, to improve outcomes for these individuals. In this role, you will be expected to work across various IT systems to gather appropriate patient information and facilitate timely discharge arrangements.

Due to the cross-organisational working between Leeds Community Healthcare NHS Trust, Leeds Teaching Hospitals NHS Trust, and Adult Social Care, excellent communication skills and networking skills will be vital to develop these relationships with colleagues, patients, and their relatives.

We are looking for individuals with experience of working within the community who have a good understanding of and are committed to delivering a Home First Approach.

This is a patient facing role and the service operates over a 7-day period, therefore weekend and Bank Holiday shifts will be required.

As an Assistant Case Manager, the main duties of your role will include:

  • Undertaking assessments with individuals on hospital wards to plan their ongoing care from hospital
  • Supporting with referrals to community rehabilitation/recovery beds
  • Supporting patients in accessing support around their property (i.e. deep cleans)
  • Linking in with Housing & Homeless teams

In this role, you will be expected to:

  • Attend daily Multi-Disciplinary Team (MDT) meetings on your aligned wards
  • Attend morning huddles within the Transfer of Care team to discuss capacity and workload
  • Conduct caseload reviews with Clinical Team Managers and Adult Social Care Team Managers

The Transfer of Care Network is an ever-changing service therefore successful candidates will need to be flexible and open to adapting to change.
