Job Description
About the Role
These new roles within our Pathway one / UEC services are being offered due to an expansion of this successful pathway. These roles will work across UEC services in North and South Nottinghamshire to provide reablement and rehabilitation to individuals.
The service supports individuals to thrive in their own environment - whether that be via preventing admissions and increasing skills to support independence or offering support and rehabilitation following hospital admission.
These roles are key to the success of the team and the therapeutic outcomes of the individuals we treat. We will work alongside Physiotherapists, Occupational Therapists and support workers to deliver the highest quality goal-focused rehabilitation.
Responsibilities
- Patient Facing Role: Delivering assessments and creating goal-focused treatment plans to maximize an individual's function.
- Caseload Management: Managing a small non-complex caseload with support from a qualified clinician.
- Record Keeping: Maintaining accurate records and supporting the wider team to safely manage reablement plans and client expectations.
- Communication: Excellent communication skills both digital and verbal and a confident manner when speaking to the public.
- Time Management: Exemplary time management and prioritization skills.

Requirements
- Knowledge and Experience: Some knowledge and experience of working with people older adults and be able to offer support/advise and care to them whilst they are treated by the service.
- Teamwork: Ability to work effectively within a team whilst being supported by registered staff.
- Autonomous Working: High degree of autonomous and lone working as this is a community role.
- Driving Licence: A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
