Assistant Practitioner - INTERNAL

Assistant Practitioner (District Nursing) – Arfon Locality

An exciting opportunity has arisen to recruit an assistant practitioner in the Arfon Locality, joining an enhanced role within the district nursing team.

Key Requirements

The successful applicant must hold the:

  • Foundation Degree in Healthcare Practice (Level 4) (essential).

About the Role

Our district nursing team provides dedicated support and care for individuals within the community across towns and rural areas. This enhanced practitioner role requires prior specialized skills outlined in the person specification.

Core Responsibilities

  • Deliver community-based nursing care to patients in their homes.
  • Work collaboratively within the nursing team to assess, plan, implement, and evaluate evidence-based care for patients.
  • Ensure timely travel between patient locations to uphold service standards.
  • Demonstrate understanding of holistic and interdisciplinary care approaches.
  • Contribute to the continuity of care for vulnerable patients, including those in isolated or challenging environments.
