About the Role
Every rehabilitation journey is different, but the right guidance can make all the difference to someone's future. As a self-employed Associate Case Manager and Occupational Therapist, you'll coordinate life-changing rehabilitation programmes, advocate for clients with complex needs and help people rebuild independence after life-changing injury.
This is an opportunity to combine specialist neurological occupational therapy with high-quality case management, working with adults and children who have acquired brain injuries, spinal cord injuries, amputations and other complex conditions. You'll manage your own caseload, work collaboratively with multidisciplinary professionals and play a central role in achieving meaningful outcomes for every client.

Your responsibilities as Associate Case Manager and Occupational Therapist
- Assess needs, developing person-centred rehabilitation plans and measurable goals.
- Coordinate multidisciplinary teams to deliver effective rehabilitation programmes and monitor progress.
- Complete risk assessments, manage budgets and ensure interventions remain appropriate and cost-effective.
- Recruit, supervise and support care staff while identifying training and development needs.
- Produce accurate clinical reports, maintain compliant records and liaise with legal representatives, healthcare professionals and external agencies.
- Continue delivering specialist occupational therapy interventions within your professional scope of practice where appropriate.
What we're looking for in an Associate Case Manager and Occupational Therapist
- HCPC registration with a recognised Occupational Therapy qualification.
- Background in neurological rehabilitation and complex clinical assessment.
- Knowledge of case management standards, clinical governance and multidisciplinary rehabilitation.
- Ability to produce high-quality clinical documentation and manage complex caseloads.
- Full UK driving licence, access to a vehicle and willingness to travel across client locations.

Working hours
Full-time or part-time, with flexibility to meet client needs, including occasional evenings or weekends where required.
