About the Role

Are you passionate about improving the lives of older adults living with dementia and mental health needs? We are looking for a compassionate, motivated and skilled practitioner to join our Older People's Mental Health Service, working as part of the ICT in Farnham.

In this rewarding role, you will provide high-quality, person-centred mental health and cognitive assessments, brief interventions, care coordination, and signposting for adults with dementia and associated mental health needs who are housebound, living in care homes, or supported within primary care. You will help ensure people receive timely support, reducing the risk of deterioration and preventing individuals from falling between services.

Working collaboratively with GPs, community teams, social care, voluntary organisations and specialist mental health services, you will use your expertise to support patients, carers and families, contribute to personalised care planning and risk management, and facilitate appropriate onward referrals where needed.

We are looking for someone who shares our commitment to delivering compassionate, high-quality care and who will uphold the Trust's values while making a meaningful difference to the lives of older people and those who support them.

Reasons to use Rodeo

I’m in my final year doing Economics and I don’t know whether to apply for grad schemes now or do a masters first. What do you think?

Honest answer — it depends on where you want to end up. A lot of top grad schemes (Big 4, civil service, banking) don’t need a masters. Let’s look at the ones you’d be competitive for now, and we can decide if a masters actually adds anything.

Also worth knowing: most autumn 2026 applications are open now. Timing matters more than you think.

Start with a chat, not a search bar

Grad scheme, placement, apprenticeship? Not sure what you want yet — that's fine. Your agent talks it through with you and turns "I have no idea" into a shortlist.
