Learning Disability Crisis Response Intensive Support Team (LD CRIST) – Band 3 Community Learning Disability Support Worker

Job Overview

The LD CRIST is a multi-disciplinary team comprising learning disability & mental health nurses, healthcare support workers, psychologists, speech and language therapists, occupational therapists, and psychiatrists.

This team operates 7 days a week during 08:00–21:00 hrs, including public bank holidays. Vacancies have arisen for a Band 3 Community Learning Disability Support Worker to join Leicestershire Partnership NHS Trust (LPT). The role supports individuals and their families in a community setting, with the primary objective of preventing unnecessary hospital admissions. LD CRIST provides rapid assessment and intensive support for people with learning disabilities, behaviours that challenge, or mental health conditions.

Senior Healthcare Assistant Role Overview

The main responsibilities include:

  • Assisting the healthcare team under the indirect supervision of registered practitioners.
  • Conducting physical or psychological measurements delegated by the team.

Job Description – Summary of Duties and Responsibilities

As a Band 3 Community Learning Disability Support Worker, your key contributions will include:

  • Assessments & Interventions

    • Undertaking direct observations.
    • Completing specialist risk assessments and care planning.
  • Wellbeing Support

    • Supporting clients/service users in maintaining health and wellbeing.
  • Clinical & Therapeutic Activities

    • Carrying out specified and delegated clinical/therapeutic tasks under supervision.
  • Healthcare Team Collaboration

    • Assisting in the planning, organisation, and delivery of clinical care with guidance from senior colleagues.
  • Safety & Equality Standards

    • Ensuring high standards of care within a safe, sensitive, and inclusive environment.
  • Implementation & Evaluation

    • Supporting registered professionals in individualised care plan implementation and evaluation.
  • Recording & Supervision

    • Accurately maintaining patient records.
    • Providing support, mentoring to colleagues and learners as appropriate.

Core Candidate Requirements

  • Experience: Prior experience supporting adults/children with learning disabilities in community settings.
  • Skills:
    • Exceptional communication/interpersonal abilities.
    • Ability to work both independently and collaboratively.
  • Legal/Logistical: UK driving licence and use of a vehicle are mandatory for role requirements.
