Job Introduction
As a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in Dartford, North Kent. This is a rewarding opportunity to deliver care where it matters most, within patients’ homes and local communities. You will support patients with a wide range of health needs, helping them to remain independent and avoid unnecessary hospital admissions. This is a varied and autonomous role where you will build strong relationships with patients and carers while developing your clinical skills in a community setting. Whether you are newly qualified or have previous experience, you will be supported to grow and develop within a friendly and collaborative team environment.

This is a full-time role working 37.5 hours per week. The service operates 7 days a week between 8am and 8pm, so flexibility is required.
Main Responsibilities
In this role, you will deliver high quality nursing care to patients in their own homes and community settings. You will work both independently and alongside the wider multidisciplinary team to assess, plan and deliver care that meets the needs of each individual. You will support patients with a range of acute and long-term conditions, promoting independence and preventing avoidable hospital admissions. This is an opportunity to develop your clinical skills while working in a more personal, patient-focused environment. You will also play an important role in communicating with families, carers and partner agencies, ensuring coordinated and effective care delivery. Key responsibilities include:
- Delivering safe, effective and compassionate nursing care in the community
- Assessing patient needs and contributing to care planning
- Monitoring patient progress and escalating concerns where appropriate
- Supporting patients and carers with information and advice
- Working collaboratively with the multidisciplinary team and external partners
- Maintaining accurate and up to date clinical records
- Contributing to service improvement and quality care delivery
Person Specification
Essential
- Registered Nurse with a valid NMC pin (Adult)
- Strong communication and interpersonal skills
- Ability to work both independently and as part of a team
- Good organizational skills and ability to manage your workload
- A caring and patient-focused approach
- Confidence using IT systems, including Microsoft Office
Desirable
- Experience within a community or primary care setting
- Experience working with patients with long-term conditions
- Understanding of admission avoidance and community-based care

Other Requirements
- Full UK driving license and access to a vehicle for work purposes
