Job Introduction
As a Band 5 Ward Nurse, you will join our valued Neuro Stroke Team on Sapphire Ward at Gravesham Community Hospital. This is a rewarding opportunity to support patients recovering from stroke and neurological conditions, helping them regain independence and achieve their rehabilitation goals. Sapphire Ward is a specialist 21-bed neurological rehabilitation unit where you will work alongside a skilled multidisciplinary team to deliver personalised, patient-centred care. You will play an important role in supporting patients throughout their rehabilitation journey, building meaningful relationships with patients and their families while making a genuine difference to their recovery and quality of life. Whether you are newly qualified or looking to further develop your clinical skills, you will join a supportive team environment where learning, development and collaboration are actively encouraged.
Package Description
You will be joining a dedicated and supportive neuro rehabilitation team who are passionate about delivering high-quality care and achieving the best possible outcomes for patients recovering from stroke and neurological conditions. The team works closely together, creating a positive learning environment where colleagues are encouraged to develop their skills, share knowledge and contribute to service improvements. We currently have a full-time opportunity available, working 37.5 hours per week.
Main Responsibilities
In this role, you will contribute to the assessment, planning, delivery and evaluation of care for patients with stroke and a wide range of neurological conditions. You will work closely with therapists, healthcare assistants, medical colleagues and the wider multidisciplinary team to deliver holistic care that supports recovery and rehabilitation.You will be responsible for maintaining high clinical standards while ensuring patients and their families feel informed, supported and involved throughout their care journey. This is an excellent opportunity to develop specialist rehabilitation nursing skills within a supportive and experienced team.Key responsibilities include:
- Delivering safe, effective and compassionate patient-centred care
- Contributing to the assessment, planning, implementation and evaluation of individual care plans
- Supporting patients throughout their rehabilitation and recovery journey
- Maintaining accurate and contemporaneous patient records
- Communicating effectively with patients, carers and members of the multidisciplinary team
- Promoting patient independence and wellbeing wherever possible
- Maintaining a safe, clean and positive clinical environment
- Working in accordance with NMC standards and professional guidelines
The service operates 24 hours a day, 7 days a week, therefore flexibility to work a range of shifts including nights, weekends and bank holidays is essential.
Person Specification
Essential
- Registered Nurse with a valid NMC registration
- Passion for rehabilitation nursing and supporting patients to maximise independence
- Excellent communication and interpersonal skills
- Strong organisational and time management abilities
- Ability to work effectively both independently and as part of a multidisciplinary team
- Compassionate and patient-focused approach to care
- Confidence using IT systems, including Microsoft Office applications

- Experience within a rehabilitation, neurology, stroke or community hospital setting
- Knowledge of caring for patients recovering from stroke or neurological illness
