About the Role
We are looking for a Band 6 nurse with experience working with Older Adults to join our Older Adult CMHT. We are a supportive, dedicated, caring team who strive to ensure we provide excellent care.
Responsibilities
Working alongside the wider MDT you will be responsible as the Care Coordinator of a caseload. This will include:
- Completing initial assessments and coordination of care needs
- Planning and implementation of evidence-based care to a defined group of older adult service users and their carers within a defined catchment area/locality, and via a job plan
- Providing professional advice and support to other agencies and members of the multidisciplinary team
- Acting up for the Hub Manager in their absence as appropriate
- Acting as a role model and resource for junior/less experienced staff
- Undertaking duty work as allocated
The Post Holder
- Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary
- Will manage a defined caseload and provide supervision to others as appropriate
- Will be responsible for the assessment, planning, implementation, and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit
- Will be responsible for ensuring effective communication pathways exist
- Will take part in the duty rota, be expected to take a senior role especially in the absence of the team manager, and to work as part of a team and help with any cover arrangements as required
- Will be responsible for ensuring the promotion of equality, diversity, and rights in accordance with good practice and legislation
- Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate
- Will participate as an active member of the duty system including the undertaking of emergency assessments
