You will provide clinical expertise to support safe, effective, and timely discharge working directly with internal and external multidisciplinary teams as required.
We are committed to providing excellent, person-centred care for our patients, who are always at the heart of what we do. Do you have the passion and enthusiasm required to join us?
The Complex Discharge Team is looking for a dynamic, innovative, and enthusiastic experienced nurse to join the team, to ensure the safe, effective, appropriate, and timely discharge of patients from hospital. You'll need to be enthusiastic and have a strong sense of purpose and good diplomatic skills in engaging staff. However, you'll also need to be tenacious, confident, calm, and persuasive. You will also be involved in the Single Transfer of Care process, undertaking triaging of referrals and decision-making regarding the hospital discharge pathways.
You will be a self-motivated team player, who will uphold and promote our Trust values. You will need to demonstrate excellent communication skills and be able to confidently liaise with all members of the multi-disciplinary teams and external agencies involved in the discharge process and provide support and teaching to the wards, attend MDT and lead and signpost on all complex discharge issues in the trust.

I’m in my final year doing Economics and I don’t know whether to apply for grad schemes now or do a masters first. What do you think?
Honest answer — it depends on where you want to end up. A lot of top grad schemes (Big 4, civil service, banking) don’t need a masters. Let’s look at the ones you’d be competitive for now, and we can decide if a masters actually adds anything.
Also worth knowing: most autumn 2026 applications are open now. Timing matters more than you think.

Grad scheme, placement, apprenticeship? Not sure what you want yet — that's fine. Your agent talks it through with you and turns "I have no idea" into a shortlist.
